Services
Eligibility for employees is based on hours worked for which an employer makes a contribution on your behalf to the Fund. You will become initially eligible on the first day of the benefit month corresponding to the eligibility month in which you first accumulate at least 125 credited hours. Your benefit coverage will start on your initial eligibility date.
Your Comprehensive Major Medical Benefit is designed to substantially cover most forms of medically necessary care. This coverage is subject to various deductibles, copayments, coinsurance, and other limits. The Blue Card PPO Network is a network of hospitals and physicians throughout the United States that participate in their local BlueCross BlueShield PPO networks.
In general, the Plan covers necessary dental expenses at 80% of the usual and customary charges for services rendered, subject to the annual deductible and maximum benefit, the treatment plan requirement, and other specific limitations. The Dental Benefit is available to all active employees and their covered dependents, as well as retirees who have chosen dental and vision coverage and their covered dependents.
This benefit helps pay for annual vision examinations and for glasses and contact lenses, subject to the rules of the Plan. The Plan uses a network of opticians, optometrists, and ophthalmologists called Vision Service Plan (VSP) to provide these benefits in an efficient and cost-effective way. Reduced benefits are available when you use a provider outside the VSP network.

Always a hassle to work with - truly gets OLD! They answer the phone and are always rude and provide horrible member service. They have no idea what they are doing in that office, you have to babysit your own claims to get them paid. You send them certified paperwork (mind you they ONLY use FAX and its 2022) and they lose over half of the FAX - then tell you that you are lying about sending it because they are too lazy to research and SEE THE DOCUMENTS were received (and I have a copy of each form sent) and still told NOPE you didnt send that.
I can EMAIL them to you, but NOPE they don't allow you to send anything in a secured encrypted EMAIL because the office is so out of date with technology and makes everything hard on the members. Really upsetting to deal with this office.

I had to have surgery that was on my gums. The oral surgeon office had confirmed that this would be covered by my insurance. When it came time to pay the bill they refused, they said it was dental. Fast forward to now I have a 6000.00 collection bill and they drained my bank account because of This 6000.00 bill!

They try everything to not pay up, and always looking to try and pass the buck on to something else( where you hurt at work, in a car wreck or at home)! Now the try to extort money from me by not paying my medical bills even though I've been maxed out for the last 6 yrs on there coverage, because they don't communicate with another insurance company and did an over payment!
But the can't tell me all the details just on medication not what kind date time how many just there trying to get there money back so the lady admitted on the phone of extorting of my medical bills! Filled with BBB, now looking at more options and maybe my lawyer!lineco is a Ponzi scheme
